4. Eligibility Criteria

Educational Qualification:

• Laboratory Technician: Bachelor’s Degree in Chemistry / Microbiology / Medical Laboratory Technology or equivalent from a recognized university + relevant laboratory experience/certification.
• Multi Purpose Health Worker (Male): 12th Science + MP HW (Male) Training Certificate from recognized institute.
• Junior Pharmacist: Bachelor’s Degree from a recognized university + basic computer knowledge.
• Female Health Worker: 12th Science + Female Health Worker (FHW) Training Certificate from recognized institute.

Age Limit (as on crucial date):

• Minimum Age: 18 years
• Maximum Age: 36 years (relaxable for reserved categories as per Gujarat Government rules)

Other Requirements: Knowledge of Gujarati and/or Hindi is essential. Candidates must be Indian citizens.

5. Application Fee

• General Category: ₹100/- + Service Charges
• SC / ST / SEBC / EWS / PH: As per Government Rules (usually exempted or reduced)

6. Selection Process

• Selection will be based on a Written Examination (Objective MCQ type) conducted through OMR or Computer Based Recruitment Test (CBRT).
• Final selection will be based on marks obtained in the written test and subsequent document verification.

5. Pay Application Fee: Pay online (₹100 for General; others as per rules) through SBI e-Pay.
6. Review and Submit: Carefully preview the form and submit. Note the Confirmation Number.
7. Take Printout: Print the filled application form and payment receipt for future reference.
